PLLA microspheres with a particle size smaller than 20um are readily recognized and phagocytosed by macrophages, after which they are cleared or degraded. Consequently, they cannot effectively reside in the tissue to continuously stimulate fibroblasts to produce collagen.

Larger PLLA microspheres especially with particle sizes >80 μm are associated with increased side effects, particularly a higher incidence of nodules. Moreover, microspheres larger than 100 μm tend to clog needles during injection, compromising the clinician's handling experience.

PLLA microspheres smaller than 20 μm carry the risk of being phagocytosed by immune cells (macrophages), which can render them unable to stimulate collagen regeneration and thus lead to unsatisfactory aesthetic outcomes.

From a clinical efficacy perspective, larger PLLA microspheres offer a greater surface area, allowing more fibroblasts to adhere. This, in turn, enhances collagen regeneration, leading to a larger volume of extracellular matrix (ECM) and consequently more pronounced aesthetic effects. However, larger particle sizes (especially >80 μm) are associated with increased side effects, particularly a higher incidence of nodules. Moreover, microspheres larger than 100 μm tend to clog needles during injection, compromising the clinician’s handling experience.

Therefore, for those seeking longerlasting aesthetic results, PLLA microspheres with slower degradation and a more extended collagenstimulation cycle are recommended. Specifically, we suggest:

EPRUI-PLLA01-30-60u (30–60 μm): suitable for deep volumetric filling and contour shaping, such as in the temples, cheeks, and jawline, where greater structural support is required.

EPRUI-PLLA01-20-40u (20–40 μm): with smaller particles and a more homogeneous suspension, this grade is ideal for superficial, finedetail injections—for example, in the periorbital and perioral areas with thinner skin, or for treating fine lines and enlarged pores.
